Please fill out this form in its entirety.  All narrative sections must be clear and concise, in plain English free of jargon or undefined acronyms. This form in part or in whole could be made public.

This form must be completed and submitted by April 15 2022—late requests will not be considered. This form and its deadline may be subject to change pending guidance from the Senate Appropriations Committee.

 Lastly, please note the following:

• This form is strictly for requests that are within the jurisdiction of the Defense Subcommittee of the Appropriations Committee. If you are submitting a request for any of the other 11 bills, please complete the General Request form.
  
  
• This form is not for congressionally directed spending items. Please use the FY2023 Congressionally Directed Spending Item Request Form for such requests.
